How Can I Become an Eye Donor?

HOW CAN I BECOME AN EYE DONOR?

-

To become an eye donor, you can request from us and then sign a Pledge Card. You must be of legal age, and you must be in the presence of two witnesses who must also be of legal age. If you are under the legal age, you must have parental permission or that of your legal guardians. Your parents may also be your witnesses.

Return the large portion of the pledge card to Lions Eye Bank of Delaware Valley (postage-paid). Make sure you keep the small portion of the pledge card with you at all times. Inform your next-of-kin that you have pledged your eyes [and perhaps any additional tissues or organs] and that you want to have your wishes carried out.

By signing an eye donor pledge card, you can help a person return to a life of independence by giving them the ultimate gift of restored or improved sight.

There are over 100 people in the Delaware Valley that are waiting for a corneal transplant. Remember, you can make a difference by signing a pledge card today!

Additionally, you can make a financial contribution to our cause. Increased funding is needed to update and expand the Lions Eye Bank's programs and activities. All donations are tax-deductable. Checks should be made out to:

Lions Eye Bank of Delaware Valley

2000 Hamilton St., Rodin Place - Ste. 301,

Philadelphia, PA 19130-3813.
