PLASMA METANEPHRINES AND CATECHOLS
There are three forms required for the Mayo Clinic plasma metanephrines and catechols testing. Be sure to request these forms when corresponding with Mayo about this testing. Your doctor should be specific about wanting these particular forms, because otherwise some may not be included. A few members have reported back that they didn't receive instruction sheets or forms from Mayo unless their doctor expressly asked for them:
1) The Instruction Form
2) The Patient Information Form
Remember that this test is available only with your doctor's consent and request, and there must be a reasonable suspicion of pheochromocytoma to have this test done in the first place.
