Stevens, and Michael J. Orchard--it is a scan of the original 1991 paper in OCR mode (optical character recognition); each individual page has been uploaded as a Gif graphics file. All United States Geological Survey publications, by the way, are in what is commonly called The Public Domain--this means that anyone can place USGS papers online without prior permission from that organization; in other words, there are no copyright infringements to worry about.
