If you're providing information on more than one spouse you'll have to fill out the form for each spouse. Please indicate the source of the information in the Comments section. Sources for this info should be things like census records, family Bibles, Birth/Death certificates, Obituaries or other official documents. If you have the capability to scan and send me photos, copies of birth, baptism. death records or other interesting Umholtz documents please do so.
